Synopsis:
It's the mid-1990s. Two young boys, one slightly older than the other are playing Hide and Seek in a patch of woods near the bay in Ocean City, Maryland. James, the younger of the two (and called "Jimmy" by his older cousin, the well-hidden Mikey) is searching frantically for the older boy. Mikey jumps out from his hiding place and frightens James. Mikey tauntingly yells "Scared?" James closes his tearing eyes. A loud voice booms in the darkness, "I asked if you were scared, Marine!" James opens his eyes. It is ten years later and he is in the Marine Corps. He shouts, "No, sir!" and then, under his breath, he adds "Not anymore." James is on his way to becoming a model Marine. He is very successful and becomes a decorated war hero.

Through James' nightmares and daydreams we learn of more about his childhood. His father, Jimmy Callera, was the head of the Callera Family in the '90s. His family runs the Mob in Ocean City, Maryland. As child he was called "Little Jimmy", but James prefers to be called James; a preference that simultaneously pleases his mother, JoAnne, and angered his father. JoAnne never wanted James to be involved in the family business. But as Jimmy Callera's only son, James' father expected "Little Jimmy" to be the heir to the Callera crime family. It was no small business either. The Mob controls the Ocean City shipping ports, as well as the Boardwalk, which, during the tourist season, is just as lucrative as the docks.

It was a time of great prosperity and, for the most part, peace. But war broke out when that peace was broken by a rival gang from Baltimore. The McMullen gang was trying to expand and their plan was to take over all of the ports in Maryland, starting with the Callera territory in Ocean City. In an attempt to send the Callera's a message, the McMullen's murdered Jimmy's younger brother, Michael. After Michael's death, Jimmy promises to care for his young nephew Mikey. Mikey and James were always close, and now they live as brothers. But where James was always an outsider to the Mob lifestyle, Mikey is all in; it's all he wants to do and be. And Jimmy seems to finally have the son that he's always wanted.

It's now present day. After two tours and eight years overseas, James heads back to the states. Two Marines walk through an airport terminal. An older couple approaches them and hugs their son, Rick McCabe. As they walk through the airport, to baggage and out to the parking lot, Rick explains to his parents that James helped him through boot camp, and also saved his life in Afghanistan. Rick's parents immediately love James. As they are all smiling and talking, a limo pulls up. Simultaneously, James phone rings. James is told that it's time to come home. He excuses himself from the conversation with Rick and his parents and heads to the limo. Rick follows James, quickly catching up and stopping him. Rick assures James that if he ever needs anything, he needs only to call.

Once back in Ocean City, James learns that all Hell is about to break loose. Jimmy is dying and leaving the family business to Mikey. It turns out that Mikey didn't only take James' place as Jimmy Callera's son either. When James talks about seeing his high school sweetheart Elaine DiGiacomo, Mikey laughs and comments on the fact that had James stuck around Mikey would have asked James to have been the best man at the wedding. Mikey also replaced James in his absence, filling the empty best friend/sidekick slot with Callera soldier, Scott Scarilira.

After Jimmy's death, Mikey becomes suspicious of everyone one, though, not without reason. Elaine is having an affair with a family gang member, Pete Santini. Santini is a vicious hit-man known as "RePete" because he always shoots his victims twice. Mikey orders a hit RePete. James, not wanting to see Elaine get hurt, requests the job. Finally thinking that James is where he belongs, Mikey gives him the job. James goes to Elaine, knowing he would never hurt her, but while there, James gets into an altercation with RePete. RePete tries to kill James, but in self-defense, James ends up killing RePete instead. While looking for RePete's identification and proof of completion of the job to bring back to Mikey, James finds a badge and detective identification on the man. Pete Santini is actually undercover cop Ken McDowell.

James finds out that Detective McDowell was on undercover assignment in order to get closer to Mikey so that he could arrest him and bring down the Callera family. It also turns out that McDowell wasn't only undercover for the OCPD. He was also on the payroll of the McMullen family, the rival gang that killed Mikey's father almost 20 years early.

War ensues between the Callera's and McMullen's with the OCPD and the Maryland State police left waiting in the wings to see who's left to arrest after cleaning up the mess. James convinces Mikey to go into hiding, after a final bloody showdown with the McMullen's. James leads Mikey to the woods near the bay where they used to hide from Jimmy when they were kids. After some debate about whether or not to turn themselves in or to fight to the death with the McMullen's and/or the police, James makes the decision for them both. After a brief standoff, James proves that, in fact, he is his brother's keeper.
